Pat Conway, RN  (Directory Founder)

Pat Conway, a nurse since 1974, has been working in the Home Health Care field since 1984.  She is pictured here with her father, who established Midwest Medical Homecare Specialists (MMHS) in Milwaukee.

Pat developed the "RN Case Manager" system for MMHS and currently works with Shelley Grohoski, HHA to implement the "Elders Support Team" concept for Homecare Matchmakers in St. Paul, MN.  She is committed to developing dementia care teams and hospice care teams in the St. Paul area.

Shelley Grohoski, HHA

Shelley holds her Home Health Aide certificate, plus a certificate in special training in dementia care.  After participating in the care of her mother , at home until her death, Shelley saw the benefit of having a support team that assists the resident as well as the family members in long term care planning and services.

Barb joins Homecare Matchmakers after years of experience as a self-employed

homemaker, companion, home health aide, in her own Business: BGS  Services.  As a Homecare Matchmakers' Associate she still works as a self-empoyed Home Health Aide, but  we do her invoice and payroll services.  Barb received her Home Health Aide license through the Twin Cities Area Chapter of the American Red Cross.
